Release Form

One form to rule them all - at least for the church. This medical authorization and release form is good for all church events until September 2022. Note: Outside event organizers may require their own release forms. Those forms can be found listed on the event page.

Sign-Up for Group Text Messages

We use Remind for text messaging both parents and youth. Texts are the easiest way to share information fast and make sure everyone (especially youth) receive it. Sign-up for free by sending the following message @mumcyouthg (don’t forget the “g”) to 81010.
